They outlined a number of things that needed to be attended to in the next cycle, and that file was, well, filed. It was an intensely frustrating experience, and I always wonder why it ends up being called &quot;lessons learned&quot;. Obviously, yes, they are lessons, but I haven&#039;t seen any evidence of them actually being learned.
